Get on the wall quicker by completing the required forms online before your visit!
Once signed, forms are valid at all Central Rock locations. Please read the following directions carefully to ensure you complete all necessary forms before visiting CRG.
All visitors must complete a Participation Agreement to check into the facility even if they are not climbing. Agreement forms for minors must be signed and completed by a Parent or Legal Guardian. Agreements cannot be signed on behalf of a minor by a grandparent, older sibling, teacher, or nanny.
If you are planning to climb at one of our Florida gyms, be sure to sign BOTH the Participation Agreement form and the Florida Under-18 Participation Agreement form (at the bottom of this page) for any visitors 17 and younger. Both of these forms are required for minors in Florida facilities in addition to the climbing orientation forms.
Watch the video here for everything you need to know to get started with Bouldering at Central Rock Gym. After watching the video, be sure to sign the acknowledgement form for all climbers and adult chaperones. Orientation forms for minors must be signed and completed by a Parent or Legal Guardian.
If you are visiting a location that offers auto belay climbing, complete this form for all climbers and adult chaperones. Orientation forms for minors must be signed and completed by a Parent or Legal Guardian. Check your specific location’s page to see what types of climbing are offered there.
For those who will be top rope climbing or belaying during their visit, please complete this form. Orientation forms for minors must be signed and completed by a Parent or Legal Guardian.
Those who will be belaying will also need to take and pass the Top Rope Belay Test administered by a CRG staff member. Not all locations offer top rope climbing, so be sure to check your specific location’s page to see what types of climbing are offered there.
This form only needs to be completed for minors who will be visiting our Florida CRG locations. The Under-18 Participation Agreement form needs to be completed in addition to the regular Participation Agreement form above. BOTH agreement forms must be completed by a Parent or Legal Guardian on behalf of any minors climbing in or spectating at our Florida locations.
Now it’s time to scale the walls. When you arrive at the gym let the front desk know that you have pre-signed your forms online.
